Most cases will require a referral from your Primary Care Physician.  Please ask your doctor to fax our office a referral.

If you have a Queens MyChart account you can request to schedule, change, or even cancel your appointment.  Once you submit this request, one of our medical assistants will reach out to you.

Please let us know as soon as possible or 24 hours in advance if you need to reschedule or cancel your appointment.

We accept MOST insurances.
